Since when is "sudden death in healthy adolescents and young adults" normal?

There's just one argument that I find vulnerable. You state that people with less healthcare contacts have more undiagnosed illnesses. One could just as easily say: they did require less health contact because they are healthier. For argument's sake I'd leave that one out, unless you have some hard evidence for that assumption. Of course there's the socio-economic component, but you already mentioned that.
